6 sigma: The Cause and Effect Diagram (C&E Diagram) is a fishbone-shaped diagram that helps a team brainstorm potential root causes of a defect. The problem to be solved is placed at the head of the fish and the six thought-generating categories - Personnel, Machines, Materials, Methods, Measurements and Environment - are placed at the ends of the fish bones.
