The Hazardous Materials Incident Reporting Requirements can be found in 49 CFR 171.15 and 171.16. There are two types of reporting requirements - immediate telephonic reporting and written detailed incident reporting. Both are based upon specific criteria. The documents below will assist you in not only determining if an incident needs to be reported, but also how to report it. Incidents can be reported either by manually completing the form listed below, or can be reported on-line. If submitting on-line, Hazmat 5800.1 incident reports are now required to be submitted using the new HAZMATICS application in the PHMSA Portal. If you have not previously signed on, you must create an account before you can report an incident on-line.
Note: DOT Incident reporting requirements are separate and different from the Solvay Incident reporting requirements. You need to check to see if you must comply with just one or both of these requirements. |
